To obtain a  sample copy, print out this form and include a check or credit card information.
For a sample copy to be mailed out of the US Send three dollars.   For a
sample copy mailed to an address in US out of Az send $2.00.  For a copy
mailed in Az send $1.00. 

A simple note with the address and method of
payment will suffice.

(three quarters) in newspaper vending racks on the sidewalk in front of
all post offices and public libraries (and about half of Basha grocery
